SECOND CHANCE HOUSING INITIATIVE

TERMS OF SERVICE AND FEE AGREEMENT

1. Purpose of Services

Upstate Apartment Rentals offers the Second Chance Housing Program to assist prospective tenants in securing affordable, flexible, or second chance housing. Services include intake, referral, landlord outreach, housing advocacy, and placement support.

2. Placement Fee

A one-time Placement Fee of $300 is due at or prior to lease signing if the client is successfully placed through the Program. This fee may be fully or partially subsidized by a participating landlord, subject to their approval. If no subsidy is offered, the full fee is the client’s responsibility.

3. Fee Conditions

The Placement Fee becomes due only upon successful placement, defined as approval for tenancy and intent to enter into a lease. Nonpayment may result in cancellation of placement or suspension from the Program.

4. Communication Consent

By applying, the client consents to receive communications from Upstate Apartment Rentals, its agents, and landlord partners via phone, text (SMS), email, or other methods. Standard message and data rates may apply. Clients may opt out by written request.

5. Confidentiality

All personal information shared during intake and referral will be used only for housing-related purposes and shared only with landlord partners or affiliated support agencies. No information will be sold or disclosed without consent, except as required by law.

6. No Guarantee of Placement

Submission of an application does not guarantee placement. Availability, housing readiness, and landlord criteria may affect eligibility and matching.

7. Voluntary Participation

Clients may withdraw from the Program at any time. However, once a placement is confirmed and a lease is in process, the Placement Fee becomes immediately due.

8. Termination of Services

Either party may terminate participation at any point. If the client accepts a lease offer facilitated by the Program, they remain responsible for the Placement Fee regardless of later withdrawal.

9. Client Acknowledgment & Consent

By signing this agreement, the client affirms:

a. All information provided is true and accurate to the best of their knowledge.

b. They consent to communications as outlined above.

c. They understand and accept responsibility for the Placement Fee upon successful placement, unless otherwise waived or subsidized.
